DURABLE LONG‐TERM REMISSION IN HIGH‐RISK RELAPSE REFRACTORY LYMPHOMA—20 YEARS’ EXPERIENCE OF ALLOGENEIC HAEMOPOIETIC STEM CELL TRANSPLANT IN SINGAPORE

Background: Allogeneic hematopoietic stem cell transplantation (AlloHSCT) is a curative option for those who have high risk relapsed/refractory lymphoma in the era of novel and cellular therapies including CAR-T cell. There paucity data evaluating long term outcomes AlloHSCT Asian patients. We performed retrospective analysis 20 years to evaluate prognostic factors patients Singapore. Methods: evaluated 121 (both non-Hodgkin (NHL) Hodgkin (HL)) from two major transplant centres Singapore had over 20-year period (2003–2022). Results: The median age 41 (range 16–71). Diagnoses include B-NHL (N = 41, diffuse large B n 21, follicular 2, mantle 6, primary mediastinal 5, chronic lymphocytic leukemia 7), T-NHL 38, peripheral T 32, other 6), NK 24) HL 18). 84% received reduced intensity conditioning (RIC), 27.3% previous transplant. Donor types matched sibling donor 54), unrelated 35), Haplo-identical donors 23) Cord blood 9). With duration follow up 56 months 0–232 months), 4-year progression free survival (PFS) overall (OS) were 38% 45% respectively. non-relapse mortality was 11.6% at day 100 24% 1 year. cumulative incidence acute graft-versus-host disease (GvHD) 36.4% with 5 having grade IV GvHD. On univariate analysis, complete remission (CR) time been given RIC improved OS. Subset analyses showed that most favourable (4-year PFS OS 61.2% 62% respectively), whereas T-NHL, lower rates 35%, 44%, 19%; 52%, 43%, 30% respectively).
